IHOT / In-House Outreach Team
BWC's In-Home Outreach Team (IHOT) is an intensive short-term outreach program.
We provide connections to supportive services within the community that are available and that work best for you. We serve all of Santa Clara County and can support our participants for up to 120 days. Our services are free, voluntary and available to adults with Medi-Cal or no insurance.
Our outreach program is designed to Advocate for you and work towards meeting your personal needs and goals.
The IHOT staff receive referrals from Santa Clara County Behavioral Health Services regarding individuals who have multiple emergency mental health service visits in the past year, recent incarceration, or other high risks. Our team will attempt to make first contact within three days.
